Automated Intake Form
In any industry documents drive business
processes. The easier documents are to create, distribute, access, exchange
and integrate, the more productive and profitable the enterprise.

Too often, chiropractic offices rely on pre-printed forms, manual data entry
or low end scanning systems to link the print job to its underlying process.
ICD automated intake form no longer has prospective patients print out a low-resolution
copy of an intake form off a doctors website and sit down and fill out the
form by hand with scribbles where mistakes were corrected. We have automated
this burdensome process. Prospective patients can now visit their doctors website
complete the intake form online or in the privacy of their own home. Once completed
an email is sent to the office staff notifying them of a new patient intake
form. Each intake form is saved into a patient management database allowing
the doctor or staff to go back and re-print the intake form at anytime.

Automated Patient Surveys
This is our flagship module,
the true powerhouse of our integrated system. The following is a list of our
automated surveys:

- RAND 36 Item Health Survey
- Progress Report
The results of each survey is automatically calculated and emailed to the
office staff notifying them of who took the survey and their score. Results
are also recorded to the patient management database. Each time the same survey
is taken and recorded the system automatically creates statistical reports
that graphically chart the patient’s progress. These charts and scores
can be printed to PDF.
The doctor can set up a schedule of how frequent each patient should take
each survey. Once the schedule is set up the system will automatically email
the patient when it is time to take the survey again. A master list to be used
by the office staff will be generated that will detail all patients who are
past due on taking specific surveys.
